Overview

This article outlines the procedures for terminating access when a contractor leaves the organization. Proper handling of these processes ensures that organizational data remains secure and that access to systems is managed effectively.


Contractor Offboarding

Contractors are not automatically offboarded through Day force. The responsibility for initiating the offboarding process lies with the contractor’s manager. This process involves submitting a request to the People team, who will then coordinate with IT to terminate access to all dependent and independent systems.

Steps:

  1. Request Submission:

    • The contractor’s manager must submit a request to the People team to terminate the contractor’s access.
    • The People team will coordinate with IT to revoke access to all relevant systems, including third-party applications.
  2. Ensure Access Termination:

    • Confirm that access to all dependent and independent systems has been successfully revoked.
  3. Prompt Action Required:

    • It is crucial that managers promptly initiate the offboarding process to prevent unauthorized access after the termination of the contractor's contract.

How To Submit a Contractor Termination Request

Follow these steps to submit a termination request for a contractor:

  1. Sign In:

    • Navigate to https://helpdesk.gohealthuc.com/hc/en-us and sign in if you haven’t already.
  2. Submit Termination Request:

    • Click on Temp/Contractor Term Request.
    • You will be redirected to a different page where the Zendesk Term Form is highlighted.
  3. Complete the Zendesk Term Form:

    • CC: Add email addresses for the impacted parties.
    • Subject (Mandatory): Enter "Terminate Contractor (Full Name)".
    • Description (Mandatory): Provide the user ID and full name of the contractor.
    • Employee Name (Mandatory): Enter the full name of the contractor.
    • Market (Mandatory): Select the appropriate market.
    • Department (Mandatory): Select the appropriate department.
    • Position (Mandatory): Provide the job title.
    • Termed/Suspended Date (Mandatory): Enter the date of termination or suspension.
    • Last Day Worked (Mandatory): Enter the contractor's last working day.
    • Term Type: Select the appropriate termination type.
    • Eligible For Rehire?: Indicate whether the contractor is eligible for rehire.
    • Forward Emails To: Specify if emails need to be forwarded.
    • List of Hardware Needed from Employee: Provide details of any hardware to be returned.
    • Special Requests/Additional Information: Add any special instructions or information.
    • Memorial Hermann Locations: Include this if relevant.
    • Attachments: Add any necessary attachments.
  4. Submit the Ticket:

    • Once the ticket is submitted, the People team will raise a ticket with the IT department to terminate the contractor's access.

  • Important: Managers must ensure the offboarding process is initiated promptly to prevent any unauthorized access after the contractor's last working day.
