If you need to request multiple center closures and not an entire market, please submit a separate ticket for each center
If you are submitting for yourself, please include your ROM, on-site manager, or Team Lead’s email address in the CC section.

If your center closure is 2 or less days away-
Upon submission of this form, it could take up to 60 minutes for these changes to be reflected on the website and Google. All scheduled customers will be removed from the queue and an automated SMS text message will be send informing them of the closure and prompting them to reschedule.

If your center closure is more than 2 days away-
